TAX RETURNS -WHAT WE will NEED FROM YOU

This will be sent to you via email at the time of booking the tax preparation service .

When you receive this it is important to fill it out in its entirety and submit with all other requested documentation. 

• This would include W-2s received for yourself/spouse.
• Any 1099's issued to yourself/spouse

1. 1099-MISC (typical for payments like royalties, awards, rent, medical or health care payments and so on)
2. 1099-NEC (used to report non-employee compensation such as independent contractors)
3. 1099-A (used by lenders when a property has been transferred due to foreclosure)
4. 1099-B (would be issued by a broker to report your gains/losses from transactions of stocks, securities and the like) 
5. 1099-C (is for cancellation of debt) Used for things like credit card foregiveness, as the IRS typically see cancelled debt as taxable income)
6. 1099-CAP (goes to shareholders who receive cash, stock and the like from any type of structural changes or acquisitions)
7. 1099-G (form used to report income paid to taxpayers from the government agencies such as tax refunds or unemployement payments)
8. 1099-R (for withdrawls from an individual retirement account or distributions from pension plans, annuities or profit-sharing)
9. 1099-S (reports proceeds from real estate transactions- such as sale or exchange of real estate, or income from certain royalty payments).
10. 1099-DIV (reports interest income, tax refunds, dividends, or unemployement benefits paid by financial institutions or banks)

• If you itemize deductions instead of taking the standard deduction for the year, then you must provide all receipts and documents pertaining to those itemized expenses. 
• If you pay for child care, mortgage, property taxes, educational expenses- this is all good information to have available to check tax credit eligibility.
• Proof of health insurance coverage for you/spouse/dependents.
